The Black Men Lawyers’ Association and Chicago Bar Association’s Young Lawyers Section are jointly hosting a 3-on-3 basketball tournament for attorneys and law students in the Chicagoland area.

General Terms

In consideration of being permitted to participate in the BMLA Basketball Tournament (“Event”), I hereby understand and agree to the following:
1. Voluntary Participation: I am participating in this Event entirely of my own free will and accord.
2. Release of Liability: I, for myself, my heirs, executors, administrators, and assigns, hereby waive, release, and discharge from any and all liability, claims, demands, or causes of action of any kind whatsoever arising out of or related to my participation in the Event, including, but not limited to, personal injury, property damage, medical expenses, or other loss, whether caused by the negligence of the Black Men’s Lawyers Association, its affiliates, insurers, agents, or representatives, or by any other person or entity.
3. Assumption of Risk: I understand that participation in the Event may involve inherent risks of injury or loss. I voluntarily assume all such risks and agree to hold harmless the Black Men’s Lawyers Association, its affiliates, insurers, agents, and representatives from all claims arising out of such risks.
4. Medical Treatment Authorization: In the event of an emergency, I hereby grant permission to organizers of the Event to secure medical treatment for me.
5. Photo/Video Release: I grant permission to Black Men’s Lawyers Association to use photographs, videos, or recordings of me taken during the Event for any legitimate purpose.
6. Entire Agreement: This Waiver and Release constitutes the entire agreement between me and the Black Men’s Lawyers Association concerning my participation in the Event.


BY REGISTERING FOR THIS EVENT, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD THIS WAIVER AND RELEASE AND AGREE TO BE BOUND BY ITS TERMS.
