Liability Waiver and Release
I hereby acknowledge and agree to the following terms and conditions in consideration of my participation in the outdoor workout event hosted by -MENT Fitness, in partnership with Behind Every Door and Run It Up Foundation.
Assumption of Risk
I understand that participating in physical fitness activities involves inherent risks, including but not limited to, physical injury, emotional stress, and other unforeseeable risks. I hereby voluntarily assume all risks associated with my participation in this event.
Waiver of Liability
I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release and hold harmless -MENT Fitness, Behind Every Door, Run It Up Foundation, their officers, directors, employees, agents, volunteers, and any other affiliated entities (collectively "the Released Parties") from any and all claims, demands, actions, or causes of action, which I may have or may hereafter have against the Released Parties, for any and all damages, injuries, or losses, including death, arising out of my participation in this event, whether caused by the negligence of the Released Parties or otherwise.
Medical Treatment
I authorize -MENT Fitness, Behind Every Door, Run It Up Foundation and its representatives to provide or arrange for medical treatment and transportation as may be deemed appropriate and necessary in their discretion, in the event of injury, accident, or illness during my participation. I agree to be responsible for any costs associated with such treatment and transportation.
Permission to Film/Photograph
I hereby grant -MENT Fitness, Behind Every Door, and Run It Up Foundation the irrevocable right and permission to film, photograph, or otherwise record my participation in this event. I agree that any such recordings may be used for promotional, commercial, or other business purposes without compensation to me. I waive any right to inspect or approve the final product or the use to which it may be applied.
Governing Law
This waiver and release shall be governed by and construed in accordance with the laws of the State of Texas. Any legal action or proceeding arising under this waiver and release shall be brought exclusively in the courts located in Dallas, Texas.
By proceeding with purchase, I acknowledge that I have read and fully understand this liability waiver and release form, and that I am voluntarily participating in this event with full knowledge of the risks involved. I further acknowledge that I am at least 18 years of age, or have had this document reviewed and signed by my parent or legal guardian.
